    Sermorelin — GHRH(1-29)

    Background

    Sermorelin acetate is a synthetic peptide corresponding to the first 29 amino acids of human growth hormone-releasing hormone (GHRH 1-29) — the minimum N-terminal sequence required for full GHRH biological activity. It was developed by Salk Institute researchers and Serono/EMD Serono in the 1980s.

    Mechanism

    Sermorelin binds the GHRH receptor on pituitary somatotrophs, producing pulsatile, physiological GH secretion that preserves negative feedback from circulating IGF-1.

    What is proven

    FDA-approved as Geref® in 1997 for the diagnostic evaluation of pituitary function and the long-term treatment of idiopathic growth hormone deficiency in children. The pharmacology of GHRH-mediated GH release is well-characterised.

    Sermorelin was withdrawn from the US market in 2008 for commercial reasons, not for safety concerns. It remains available via compounding pharmacies under the FDA 503A framework in the US.

    What is NOT proven

    Adult anti-ageing, body-recomposition, or sleep-quality benefits in healthy adults are not supported by completed pivotal trials. Most adult-use protocols extrapolate from paediatric GH-deficiency efficacy and physiological GH-axis pharmacology.

    Open research questions

    Long-term IGF-1 and oncological surveillance in adult off-label use; comparative efficacy versus selective GHRPs (Ipamorelin) and longer-acting GHRH analogues (CJC-1295 No DAC, modified GRF 1-29).

    Regulatory status

    Previously FDA-approved as Geref® (paediatric GH deficiency); commercial product withdrawn 2008. Compounded sermorelin remains available in the US under 503A pharmacy compounding. Not approved as an anti-ageing or body-recomposition therapeutic anywhere.
