Kisspeptin-10 — Reproductive Axis Regulator

Background
Kisspeptin-10 (KP-10) is the C-terminal decapeptide of kisspeptin (formerly metastin), a 54-amino-acid peptide encoded by the KISS1 gene. Kisspeptin signalling through the GPR54 receptor is the upstream regulator of GnRH-secreting neurons in the hypothalamic-pituitary-gonadal (HPG) axis.
Loss-of-function mutations in KISS1 or GPR54 cause hypogonadotropic hypogonadism in humans, establishing kisspeptin as essential for normal reproductive function.
Mechanism
Kisspeptin binding to GPR54 on hypothalamic GnRH neurons triggers GnRH release, which in turn drives pituitary LH and FSH secretion, and ultimately gonadal sex-steroid production.
What is proven
Imperial College London (Dhillo, Comninos, and colleagues) and other groups have published multiple human studies showing reliable, dose-dependent LH and FSH release following kisspeptin administration in healthy men and women.
Kisspeptin has clinical utility as a diagnostic agent in hypothalamic amenorrhoea and as a trigger for oocyte maturation in IVF cycles (early-phase trials).
fMRI studies (Comninos et al., Journal of Clinical Investigation 2017; subsequent work) suggest kisspeptin modulates activity in limbic brain regions associated with sexual and emotional processing.
What is NOT proven
Routine long-term use of kisspeptin-10 for libido enhancement, fertility, or mood is not characterised in completed Phase 3 trials. No therapeutic kisspeptin product is currently approved for these indications.
Open research questions
Optimal dosing regimen for IVF oocyte maturation versus current hCG triggers; whether central nervous system effects translate to durable mood or libido benefit; long-term safety with chronic dosing.
Regulatory status
Not approved as a therapeutic for any indication by any major regulator. Investigated in registered clinical trials at multiple academic centres.
