BPC-157 — Body Protection Compound

Background
BPC-157 (Body Protection Compound 157) is a synthetic 15-amino-acid sequence (Gly-Glu-Pro-Pro-Pro-Gly-Lys-Pro-Ala-Asp-Asp-Ala-Gly-Leu-Val) corresponding to a stable fragment of a larger protein identified in human gastric juice in the early 1990s by Sikiric and colleagues at the University of Zagreb.
It is one of the most-cited 'repair peptides' in research literature, yet remains unapproved by any major regulator for any indication. The World Anti-Doping Agency added BPC-157 to its prohibited list in 2022.
Mechanism
Proposed mechanisms in the published literature include upregulation of VEGF-R2 (driving angiogenesis), modulation of the nitric oxide (NO) system, interaction with the dopaminergic and serotonergic systems, and promotion of FAK-paxillin and early growth response 1 (EGR-1) signalling in tendon fibroblasts. These are mechanistic hypotheses based primarily on rodent and cell-culture work.
What is proven
In preclinical models (rats, mice, in-vitro tissue), BPC-157 has been shown to accelerate tendon-to-bone and ligament healing after surgical transection, protect gastrointestinal mucosa against NSAID- and alcohol-induced ulceration, and improve angiogenic outcomes in ischaemic models. These findings have been replicated by multiple research groups internationally.
Acute toxicity in rodents is reported as very low across the ranges examined in published work.
What is NOT proven
There are no completed Phase 2 or Phase 3 human efficacy trials for BPC-157. Long-term safety in humans and any specific disease-modifying effect in humans are unestablished.
Claims circulating in fitness and wellness markets — that BPC-157 'heals any injury', restores cartilage, or treats post-concussion syndrome — are not supported by controlled human data.
Open research questions
Bioavailability and pharmacokinetics in humans remain poorly characterised. Whether the rodent angiogenic and tendon-healing effects translate to humans at all is unknown. Long-term oncological signal (given growth-factor pathway involvement) has not been formally studied.
Regulatory status
Not approved by the FDA, MHRA, EMA, TGA or Health Canada for any indication. Banned in competition sport by WADA since 1 January 2022. In the United States, the FDA placed BPC-157 on the 503A bulk substances 'Category 2' list in 2023, restricting compounding pharmacy use.
